A consultation with a OMMA Board-Certified Physician

A copy of your recommendation form

A digital receipt of your patient fee

Affordable Pricing:

New patients, minors, and renewals: $49

State fee: $104.30

For SoonerCare, Medicare, Medicaid, or 100% Disabled Veterans, the state fee is $22.50

We also offer $25 Application Assistance for patients who need help completing the online state application.
